I recently purchased a bulgarian mak and after shooting it this weekend I noticed the grip screw is missing.
What kind of grip screw do I need? The screw from my russian does not fit.
I see on mak.com that there are two types a wide and a narrow. Is there also some type of sleeve that goes around the narrow screw?
Any help would be appreciated.

They are the same thread. The Russian has a larger diameter head and the Bulgy and others have a smaller head.
The smaller screw you need is a "notched" style and is for the red bakelite grip that comes with the Bulgy. It has notches that catch on a metal catch in the grip itself.
My guess for this design is you can adjust the grip screw to the desired tension and it wont back out.
You can call Makarov.com and they can set you up with the proper screw.

There are actually several variations of grip screw. The std issue Russian is the fluted head, used on the Russian and Bulgarian red star grips. The EG grips came in two styles: "old style" used the same fluted head, "new style" used a wide head screw. The Simson-Suhl and the Norinco grips used variations thereof.
The std thread is the same on all: 5mm. The screw length may vary between styles. If you are in a pinch, go to the hardware store and buy some 5mm cheese head, pan head or socket head screws from 10-14mm in length and try them out. You can always cut them to length.
If you post the grip, someone may be better able to help.
